29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a.    On or about January 8, 2019, on the roof, employees operating pneumatic nailers to fasten roof shingles were exposed to eye injury hazards from flying particles in that they were not provided protective safety glasses.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1926.102(a)(1), which was contained in OSHA inspection number 1207831, Repeat Serious citation number 2, item number 1, and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 30 Milliken Lane, Saint Johns, FL 32080.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1926.102(a)(1), which was contained in OSHA inspection number 997948, Repeat Serious citation number 2, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 228 Melissa Ray Drive, Jacksonville, FL 32225. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. Employees performing roofing work are not protected from fall hazards by the use of a fall protection system.  Travis Slaughter d/b/a Florida Roofing Experts fails to assure effective protections against falls as required by the standard, as demonstrated on January 8, 2019, at a job site at 4056 Lonicera Loop, Fruit Cove, Florida, when 10 roofing employees working on the 6:12 pitch slopes of first-story and second-story roofs were exposed to 9 feet to 26 feet fall hazards from the roof eaves and the raked edges of the peaks of roof gables in that they were not protected by a fall protection system.  Travis Slaughter d/b/a Florida Roofing Experts, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1318779, Willful citation number 1, item number 1, and was affirmed as a final order on December 14, 2018, with respect to a workplace located at 1116 Nochaway Drive, St. Augustine, FL 32092.  Great White Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1251412, Willful citation number 1, item number 1, and was affirmed as a final order on October 23, 2017, with respect to a workplace located at 1601 University Blvd. North, Jacksonville, FL 32211.
