29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.     On or about April 26, 2018, during the removal and installation of roofing material of a multi condominium building located at 25863 Kensington Drive, Westlake, Ohio, the employer did not ensure employees using ladders to access and egress from the roof were trained to recognize ladder hazards and the procedures to follow to minimize ladder related safety hazards.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.     On or about April 26, 2018, during the removal and installation of roofing material of a multi condo building located at 25863 Kensington Drive, Westlake, Ohio, the employer did not ensure an employee exposed to a fall hazard of approximately 9 feet was trained to recognize fall hazards and the procedures to follow to minimize fall hazards.           CHARLES CHAPMAN CONSTRUCTION, LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.503(A)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1047647, CITATION NUMBER 01, ITEM NUMBER 001C AND WAS AFFIRMED AS A FINAL ORDER ON OR ABOUT APRIL 14, 2015, WITH RESPECT TO A WORK PLACE LOCATED AT 1239 SHAFFER DRIVE, LORAIN, OHIO.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.     On or about April 26, 2018, the employer did not report the in-patient hospitalization injury of an employee, who sustained injuries related to a fall from a residential roof while performing roofing construction activities. The employee was admitted to the hospital for treatment of head fracture, rib fracture and other internal injuries. OSHA was not notified until approximately May 25, 2018.
