29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation: a) On or about May 24, 2018, the employer had inaccurately completed the OSHA 300 log for entry #3 for the log provided on June 21, 2018. The log showed the employee was away from work for 3 days. The employer Med-Call log for entry #3 showed the removal would be for 6 to 8 weeks.
