[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995]     a) On or about June 28, 2018: Three employees of Big Hawk MX Builders LLC were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 9-feet fall hazard.     b) On or about June 28, 2018: Two employees of Big Hawk MX Builders LLC were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 20-feet fall hazard.    c) On or about June 28, 2018: An employee of Martin Serrano Remodeling LLC was installing roof trusses and was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, employee installing the roof trusses was exposed to a 9-feet fall hazard.    d) On or about June 26, 2018: Three employees of Elite Construction & Associates LLC were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 9-feet fall hazard.    e) On or about June 26, 2018: Four employees of Elite Construction & Associates LLC were observed installing roof joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 9-feet and 32-feet fall hazards.     f) On or about June 27, 2018: Four employees of Elite Construction & Associates LLC were installing roof sheathing and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 11-feet and 34-feet fall hazards.     g) On or about June 28, 2016: Four employee of Elite Construction & Associates LLC were observed installing a parapet wall and covering some roof openings that were left from the previous day were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee provided with an alternative fall protection measure, the employees were exposed to a 34-feet fall hazard.     h) On or about June 28 2016: Two employees of Elite Construction & Associates LLC were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure, the employees were exposed to a 9-feet fall hazard.
