29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Villa Construction LLC Located at 5291 Walden Street, Denver, CO 80249:  On and preceding July 14, 2018, the employer did not ensure that employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects. Two employees working at ground level were assisting with material preparation directly below a worker utilizing a pump jack scaffold installing siding.  The workers on the ground level were not utilizing head protection.  This exposed the employees to struck-by hazards from falling materials.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of a personal fall arrest system or guardrail system meeting the requirements of paragraph (g)(4) of this section:  a) Villa Construction LLC Located at 5291 Walden Street, Denver, CO 80249:  On and preceding July 14, 2018, the employer allowed the use of a pump jack scaffold that was missing the mid-rails and end-rails, while the employees were siding a two-story single family home under construction. The condition exposed employees to fall hazards of up to 20 feet.  Villa Construction LLC was previously cited for a violation of is occupational safety and health standard or its equivalent 29 CFR 1926.451(g)(1)(vii) which was contained in OSHA inspection number 1069251, citation number 1, Item 2, issued June 24th, 2015 and became a final order on July 1, 2015, with respect to a workplace located at  4435 Walden Court, Denver, CO 80249.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) Villa Construction LLC Located at 5291 Walden Street, Denver, CO 80249:  On and preceding July 14, 2018, the employer did not ensure that employees were protected from falling when installing siding on the east side of the house above the second floor.  The employee used a pump jack scaffold to access a this section of roof above the second floor of a residential house to install siding. This condition exposed employees to fall hazards of up to of up to 20 feet.  Villa Construction LLC was previously cited for a violation of is occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1069251, citation number 1, Item 3a, issued June 24th, 2015 and became a final order on July, 1, 2015, with respect to a workplace located at  4435 Walden Court, Denver, CO 80249.
