29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Plant - On or about 08/21/18 and at times prior thereto employees operated the bench pneumatic press to install bushings into metallic parts. The machine was not guarded, exposing workers to pinch-points created by the ram.    29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Plant - On or about 08/21/18 and at times prior thereto employees operated the bench pneumatic press to install bushings into metallic parts. The machine was not guarded, exposing workers to pinch-points created by the ram.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  a) Plant - On or about 08/21/18 and at times prior thereto employees applied lockout/tagout methods during maintenance/repairs of equipment including (but not limited to) presses, robots, rimming machines, spot welders, and drill presses. The employer did not certify that the inspections/audits of the specific lockout/tagout procedures were conducted and maintained to date.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
