29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On October 24, 2018, Chester Mast dba Mast Construction was issued citations and the employer did not document that each citation was abated.  Abatement certification was not received for the following violations:       a.    Citation 1 Item 1, and  b.    Citation 1 Item 2a, and c.    Citation 1 Item 2b

29 CFR 1926.451(f)(7):  Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:     On or about September 14, 2018, Employees were exposed to a fall hazard on a construction site in Champaign, Illinois, and the elevated platform attached to the rough terrain, was not erected and moved under the director of a competent person before employees were allowed to access the scaffold.
