29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On October 18, 2019, Chester Mast dba Mast Construction, was issued citations and the employer did not verify, through documentation that each citation was abated.  Abatement documentation was not received for the following violations:        a.    Citation 1, Item 1     b.    Citation 2, Item 1

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about August 13, 2019, employees were exposed to a fall hazard up to 9 feet while framing walls on the third story of a commercial building and suitable fall protection was not used.     b. On or about August 13, 2019, employees were exposed to a fall hazard up to 18 feet while installing walls on the third story of a commercial building and suitable fall protection was not used.  CHESTER MAST DBA MAST CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD: 29 CFR 1926.501(B)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1260578, CITATION NUMBER 1,  ITEM NUMBER  1(A), AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 13, 2017  WITH RESPECT TO A WORKPLACE LOCATED AT 302 S 1ST STREET CHAMPAIGN, ILLINOIS 61820.
