29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a.) On or about October 25, 2018, the employer did not assure that employees were adequately trained in the lockout procedures for including, but not limited to the #6 CNC 2 SMT Industries Tube Bender in the shop area. Employees did not lock out and tag out the electrical panel while changing out blocks and arbors on the tube bender exposing employees to crush hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about October 25, 2018, employees were exposed to amputation injuries in that the #6 CNC Tube Bender tear down band saw was not locked out prior performing change over activities, including but not limited to arbor and block changes. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to maintenance task - the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) On or about October 22, 2018, the employer did not ensure employees were protected while operating the #6 CNC 2 SMT Industries Tube Bender. An employee was attempting to wipe lube from the tube to protect against part deficiencies during a cycle, when his finger became caught in between the tube and the pressure die, resulting in a partial finger amputation.
