29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:    On or about November 15, 2018, the employer did not ensure all employees required to wear half-faced tight fitting respirators for work tasks such as spray finishing had an annual fit test.      Forestville Builders & Supply, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1233497, citation number 1, item number 2c and was affirmed as a final order on November 15, 2017, with respect to a workplace located at 141 Main Street in Forestville, WI.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.  On or about November 15, 2018, in the 146 Main Street building, the employer did not ensure that faceplate for box mounted switch that operates the dust collection system above the Delta Unisaw completely covered the opening in that the faceplate was broken.
