29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation.  On or about December 12, 2018, employees using mechanical equipment did not have flags installed 10 feet from the roof edge, exposing employees to a fall of 14.9 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about December 12, 2018 employees performing roofing operations on flat roof were not protected from a fall, exposing employees to a fall of 14.9 feet.    Premier Roofing Design, Inc. DBA Top Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(f)(1)(I), which was contained in OSHA inspection number 1290156, citation  number 2, item number 1 and was affirmed as a final order on April 23, 2018 with respect to a workplace located at 5706 8th Ave., Kenosha, WI 53140.    Premier Roofing Design, Inc. DBA Top Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 995814, citation number 01, item number 01 and was affirmed as a final order on July 13, 2015, with respect to a workplace location at 911 W. 50th Place, Chicago, IL 60609.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    On or about December 10, 2018, an employee was hospitalized after falling through a roof while performing roofing operations.  The employer failed to report the hospitalization within 24 hours.     No abatement certification or documentation is required for this item.
