29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      On or about December 12, 2018, Estad Stamping & Mfg. Co. had employees perform maintenance operations including die changes/adjustments and clearing jams on equipment, including, but not limited to: mechanical power presses and press brakes, and the employer did not develop, document, and utilize an energy control procedure for each piece of equipment.  These energy control procedures would include the following:      a.         A specific statement of the intended use of the procedure,  b.         Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy,   c.         Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and  d.         Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:             On or about December 12, 2018, Estad Stamping & Mfg. Co. had employees perform maintenance operations including die changes/adjustments and clearing jams on equipment, including, but not limited to: mechanical power presses and press brakes, and the employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls was acquired by employees.  This training would include the following:       a. Authorized employees - recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control,   b. Affected employees - purpose and use of the energy control procedure, and   c. Other employees - instructed about the procedure and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked or tagged out.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about December 12, 2018, Estad Stamping & Mfg. Co. had employees perform maintenance operations including die changes/adjustments and clearing jams on equipment, including, but not limited to: mechanical power presses and press brakes, and the employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
