The most recent occurrence was evident during the inspection conducted on January 30, 2019 at the facility located at 4433 S. Jurgen Rd, Grand Island, NE 68801.  The employer has employees using an overhead crane to lift and move wooden trusses that has not been properly marked with the rated load on each side.

Employees are using a self-made under the hook device that has not been rated or certified by an engineer.  Employees lift wooden trusses that weigh approximately three to five hundred pounds.

No guard is available to protect employees from the rotating chuck when the drill press is used to drill holes in wooden 2 inch by 4 inch pieces.

This was most recently evident during an inspection conducted on January 30, 2019 at the facility located at 4433 S. Jurgen Rd, Grand Island, NE 68801.  The employer is using a flexible extension cord in place of fixed wiring to provide power to a drill press.  This configuration has been in place for approximately one year when drill press was purchased.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   The employer is failing to conduct annual periodic reviews of the written specific procedures used as part of the company lockout and tag out (LOTO) program where employees are exposed to electrical shock, caught in, and amputation hazards.  The most recent occurrence was evident during the inspection conducted on January 30, 2019 at the facility located at 4433 S. Jurgen Rd, Grand Island, NE 68801.  The employer has written LOTO procedures that are utilized by sub-contractors when maintenance is performed on the various wood and metal cutting machines.  The procedures have not been formally reviewed within the past year as outlined in this subpart.
