NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

On or about January 23, 2019, at 1039 Gulf Drive, Summerland Key, Florida, 33402, the employer did not ensure that employees who were engaged in roofing activity were adequately trained to:  a) Recognize when equipment is damaged and needs to be removed from service;  b) How to calculate fall distances when using a personal fall arrest system; and  c) Recognize the hazards of working on a roof without a personal fall arrest system and/or the need to be connected to an anchor point. 29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about January 23, 2019, at 1039 Gulf Drive, Summerland Key, Florida, 33402, an employee was exposed to a 20-foot fall hazard when applying roofing materials/self-adhering base sheets (peel and stick) onto a steep slope roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, and was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems.     Justification for REPEAT: LINDHOLM CONSTRUCTION INC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), or its equivalent standard, which was contained in OSHA inspection number 1106374, citation number 01, Item number 002, and was affirmed as a final order on 02/17/2016, with respect to a workplace located at 82779 Overseas Hwy, Islamorada, FL 33036.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
