29 CFR 1926.502(d)(19): Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and/or were reused before they were inspected by a competent person and determined to be suitable for reuse:    On or about January 29, 2019, at 800 Georgia Street, Key West, Florida, 33040, an employee was exposed to an 11-foot fall hazard when applying roofing materials on an low sloped (4:12) roof, while wearing a Personal Fall Arrest System, wherein the lanyard was previously subjected to impact loading and was not immediately removed from service.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  On or about January 29, 2019, at 800 Georgia Street, Key West, Florida, 33040, the employer did not ensure that the personal fall arrests systems used by employees who were applying roofing materials on a low sloped (4:12) roof were inspected, in that employees were using a Personal Fall Arrest System in which one of the lanyards was previously subjected to impact loading, and three of the lifelines were observed damaged; therefore exposing the employees to an 11 foot fall hazard.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  On or about January 29, 2019, at 800 Georgia Street, Key West, Florida, 33040, the employer did not ensure that employees who were engaged in roofing activity were adequately trained to:  a) Recognize when equipment is damaged and needs to be removed from service. b) How to calculate fall distances when using a PFAS. c) Recognize the hazards of working on a roof without a Personal Fall Arrest System and/or the need to be connected to an anchor point.  NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about January 29, 2018, at 800 Georgia Street, Key West, Florida, 33040, employees were installing sheathing/self-adhering base sheets on a low-slope roof, less than 50-feet in width, 6 feet (1.8 m) or more above lower levels, without a guard-rail system, safety net system, or Personal Fall Arrest System (PFAS), or without a fall protection plan which meets the requirements of paragraph (k) of 1926.502; therefore exposing the employees to 11-foot fall hazards.     Justification for FIRST REPEAT: LINDHOLM CONSTRUCTION INC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), or its equivalent standard, which was contained in OSHA inspection number 1106374, citation number 01, Item number 002, and was affirmed as a final order on 02/17/2016, with respect to a workplace located at 82779 Overseas Hwy, Islamorada, FL 33036.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
