29 CFR 1926.761(c)(3): Where controlled decking zones (CDZ) are being used, the employer did not assure that each employee had been provided training in the nature of the hazards associated with work within a CDZ and the establishment, access, proper installation techniques and work practices required by �1926.760(c) and �1926.754(e):    a) 1 Hog Island Rd., Philadelphia, PA 19153 - The employer did not ensure that an employee who was working on the leading edge of a controlled decking zone at a height of approximately 29 feet above the lower level was trained in the nature of the hazards associated with work within a CDZ and the establishment, access, proper installation techniques and work practices required by �1926.760(c) and �1926.754(e), thus exposing the employee to fall from height hazards.  This condition was noted on or about February 11, 2019.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a) 1 Hog Island Rd., Philadelphia, PA 19153 - On or about February 11, 2019, an employee was hospitalized due to a work-related injury.  The employer did not report the fatality to OSHA within 24 hours of becoming aware of the employee's in-patient hospitalization.
