29 CFR 1910.28(b)(1)(i):  Walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was not protected from falling by one or more of the following:      a.  On or about March 8, 2019, at the building adjacent to the shear, employees were exposed to fall hazards of approximately six feet three inches (6'3") to the stairway when accessing the control room of the sheer.      b.  On or about March 8, 2019, on the platform adjacent to the shear, employees were exposed to fall hazards of approximately eleven feet 6 inches (11'6") when accessing the shear platform during unjamming of shear and maintenance tasks.
