29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Grinding Department: On or about March 11, 2019, and at times prior, employees were exposed to unexpected energization while clearing a jam from a Cincinnati Centerless Grinder LR 220-8 (serial number M110-002-0018).

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Grinding Department: On March 11, 2019, and at times prior, employees were exposed to unexpected energization while performing servicing and maintenance operations on equipment including, but not limited to, Cincinnati Centerless Grinder LR220-8. The employer failed to conduct periodic inspections of the energy control procedure.
