29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).     On or about March 28, 2019, at a job-site located at 5894 Reynolds Rd, Mentor-On-The-Lake, Ohio 44060; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls with a depth of ranging from approximately 5.6 to 6 feet.
