29 CFR 1917.11(a): Active work areas were not kept free of equipment, materials, debris, protruding nails, and other sharp objects not necessary for the work in progress. a) On or about April 08, 2019 an employee operating a GMC Savana van (converted into a utility truck), ID number 103 was involved in an explosion. The cab area of the truck was full of combustible debris such as: clothing (including boots and shoes), rags; plastic bags and trash, water bottles, cardboard boxes and paper, disposable gloves, equipment,truck parts and food. The magnitude of the items found in the cab was such that it could have ignited during the fire, placing the driver and others at greater risk.

29 CFR 1917.152(d)(1)(vi): Compressed gas cylinders did not have valves closed when cylinders were empty, being moved or stored. a) Worksite: On or about April 8, 2019 an employee was transporting/moving equipment, including an oxygen and acetylene compressed gas cylinders in the back of a GMC Savana van/truck. The cylinders were determined to have contributed to an explosion which occurred while the truck was returning to the laydown yard. Both cylinder valves were determined to have been in the open position at the time of the explosion.

29 CFR 1917.152(d)(1)(xvi): Oxygen cylinders were not separated from fuel gas cylinders and combustible materials by either a minimum distance of 20 feet (6.1 m) or a barrier having a fire-resistance rating of 30 minutes. a) Worksite: On or about April 8, 2019 an employee was transporting/moving equipment, including an oxygen and acetylene compressed gas cylinders in the back of a GMC Savana van/truck. The cylinders were determined to have contributed to an explosion which occurred while the truck was returning to the laydown yard. The cylinders had not been used in 4 days and should have been placed in storage where they were separated by 20 feet or had a fire resistant barrier between them.
