On or about May 6, 2019, the employer failed to secure the LPG tank on the Caterpillar #10, Model GC45K-SWB, S/N AT8701011, forklift which had a broken strap.  b. On or about May 6, 2019, the employer failed to secure one of the two LPG tanks, located to the back left side on the Royal #11 Model TA220B, S/N LO856 forklift which had a broken strap.
