29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about May 6, 2019, the employer failed to ensure that employees loading slave pallets on the stacker, located on the CTL Line, were  exposed to a crushing and struck by hazard from the inadequately guarded equipment. There was a gap between the stacker and frame of approximately 38.5 inches.  Employees perform stacking in this area and are exposed to a crushing hazard between the stacker and frame as it moves in and out of the equipment. The employees are also exposed to a struck by hazard as nothing prevents the employees from entering the loading/unloading area as the stacker moves in and out of equipment.
