Greto Heartland Industrial Packaging, LLC - Lima, Ohio: On or about May 1, 2019, employees that operated the SR1 slitter line were exposed to a nip point and rotating part hazards in that the end of the unwind shaft on the back stand, located on the infeed side of the machine, was not guarded.    b. Greto Heartland Industrial Packaging, LLC - Lima, Ohio:On or about May 14, 2019, employees that operated the SR3 slitter line were exposed to nip point and rotating part hazards in that the ends of the unwind shaft on the back stand, located at the infeed side of the machine, were not guarded.

Greto Heartland Industrial Packaging, LLC - Lima, Ohio: On or about May 7, 2019, the employer did not ensure that each authorized employee affixed a lockout device(s) to energy isolating devices of the SR3 Kidder slitter machine.  Employees that setup the machine were exposed to the unexpected start-up of rotating parts.  b. Greto Heartland Industrial Packaging, LLC - Lima, Ohio: On or about May 13, 2019, the employer did not ensure that lockout devices were affixed to the energy isolating device(s) of the SR4 Appleton slitter machine.  Employees that setup the machine were exposed to the unexpected startup of rotating parts.    c. Greto Heartland Industrial Packaging, LLC - Lima, Ohio: On or about May 1, 2019, the employer did not ensure that each authorized employee affixed a lockout device to the energy isolating device for the drive chain on the carpet roller's re-winder.  Employees that replaced the chain were exposed to the unexpected startup of rotating parts.   Greto Heartland Industrial Packaging, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(4)(i) which was contained in OSHA inspection number 1248141, citation number 1, item number 2a and was affirmed as a final order on September 19, 2017, with respect to a workplace located at 1025 Findlay Rd., Lima, Ohio.
