29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level.   At the part washer, the stairway on one side consisted of a top rail and post but no intermediate rail and next to the hot part washer wall stairway consisted of no top rail or intermediate rail, exposing employees to fall and burn hazards.

29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping or stepping into or through any hole that is less than 4 feet above a lower level by covers or guardrail systems.   At the part washer above the hot chemical tank, the employer did not ensure that grates were secure to prevent employees from falling, tripping or stepping into the chemical tank below, exposing employees to burn hazards.
