29 CFR  1910.253(b)(2)(iv):Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:  On or about November 10, 2022 and at dates prior thereto for employees exposed to fire and struck-by hazards where an acetylene cylinder was placed into storage without it's valve cap in place.

29 CFR  1910.252(c)(6)(ii):Indoor welding or cutting involving zinc-bearing base or filler metals coated with zinc-bearing materials was not done in accordance with 29 CFR 1910.252(c)(3):  On or about November 10, 2022, and at dates prior thereto, for employees exposed to respiratory hazards from zinc oxide fumes while welding galvanized steel, where the employer did not provide a compliant exhaust ventilation system.  NOTE: Champion Cooler  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.252(c)(6)(ii) which was contained in OSHA inspection number 1603077, citation number 1, item number 1 and was affirmed as a final order on September 6, 2022, with respect to a workplace located at 1724 South Scullin Avenue, Denison, Texas.
