29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) Best Decision Stucco Corp. at 2919 W 23rd Ave. Denver, CO 80211: On or before July 17, 2019, the employer did not protect employees from injury, in that the first and third working level platforms were not fully planked. This condition exposed employees to fall hazards in excess of 7 feet when working from the fabricated frame scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) Best Decision Stucco Corp. at 2919 W 23rd Ave. Denver, CO 80211: On or before July 17, 2019, the exposing employer did not ensure that employees were protected from a fall from elevation, in that guardrails or other suitable fall protection were not properly installed on the working platforms of the fabricated scaffolding. The scaffold lacked endrails all four levels and a midrail on the fourth level. This condition exposed employees to fall hazards ranging from 35 feet to 10 feet.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: (a) Best Decision Stucco Corp. at 2919 W 23rd Ave. Denver, CO 80211: On or before July 17, 2019, the exposing employer did not ensure that employees were protected from a fall from elevation, in that several footings of the fabricated frame scaffold were placed onto wood that was bending due to the weight of the scaffold. One footing was secured to a mud sill that was positioned halfway off of a concrete walkway. This condition exposed employees to fall hazards ranging from 35 feet to 10 feet.
