29 CFR 1910.303(g)(2)(i)(D): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by elevation of 2.44 m (8.0 ft) or more above the floor or other working surface:  Dry Dock 3, Polar Star: A light switch was energized and was attached to a metal electrical box with knockouts was about 2 inches from the ground and exposed to wet conditions. Employees were exposed to electrical hazards.

29 CFR 1915.112(b)(1)(i): 29 CFR 1915.112(b)(1)(i):  Employer did not ensure wire rope and wire-rope slings have permanently affixed and legible identification markings as prescribed by the manufacturer indicating the recommended safe working load for the type(s) of hitch(es) used.  Dry Dock 3, Polar Star Ship: There were two instances where damaged wire ropes were used by employees to lift equipment that weighed several hundred pounds and legible load capacity information was not provided. Employees were exposed to struck by and crushing hazards.
