Employees working on the roof were disposing of roofing debris by throwing it from the roof to the ground and dumpster below. Employees working on the ground floor were exposed to struck-by hazards from the material being thrown from above.

The portable ladder that the employees were utilizing at the jobsite had a broken rung. The portable ladder was not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.

All American Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1378815, citation number 1 item number 2 and was affirmed as a final order on 04/26/2018, with respect to a workplace located at 103 Southeast 291 Highway, Lees Summit, Missouri 64063.  All American Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1302016, citation number 1 item number 1 and was affirmed as a final order on 06/25/2018, with respect to a workplace located at 920 Columbus, Lees Summit, Missouri 64086.

All American Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1369637, citation number 1 item number 3 and was affirmed as a final order on 04/26/2018, with respect to a workplace located at 112 Missouri 291, Lees Summit, Missouri.
