29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):     On or about September 24, 2019, the employer did not ensure the Black Max Link Lite light curtain point of operation guard was properly applied for every operation performed on the 400 Ton Komatsu Mechanical Power Press, Model Number E2G400U-D.

29 CFR 1910.217(c)(3)(iii)(f): Mechanical power press(es) using presence sensing point of operation devices were not provided with guards to protect all areas of entry to the point of operation not protected by the presence sensing device(s):     On or about September 24, 2019, an employees feeding stock into a 400 Ton Komastu Model Number E2G400U-D Mechanical Power Press was not protected at all areas of entry by a guarding device.   The placement of a light curtain created an approximately 15 inch area of entry between the base of the machine and the start of the bottom of the light curtain.
