On or about 10/17/2019, at the job site - On 10/17/2019 at 10:47A.M. the employer was requested to provide OSHA 300 logs, but the employer failed to provide copies of work-related injuries and illnesses or OSHA 300 logs within 4 hours of request. On 10/17/2019 at 5:29 P.M., the employer submitted the 2019 OSHA 300 only.
