29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).      On or about January 13, 2020, at an excavation site located South of N.W. 150th on Serenita Ave. in Oklahoma City, OK, no protective devices were in place to protect an  employee working in a trench that was more than 5 feet in depth, exposing the employee to the hazard of being crushed by falling soil.         Jerlow Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection numbers 1411703, citation number 1, item number 2, and was affirmed as a final order on November 7, 2019, with a final abatement date of October 18, 2019, with respect to a workplace hazard located at 3100 Covell in Edmond, OK 73025.
