On or about February 7, 2024, and at times prior thereto, while installing water lines for a new housing community, employee 1, Laborer, was exposed to soil collapse when required to enter an unprotected trench and was not trained to recognize and avoid unsafe conditions when working in trenches.

29 CFR  1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.   On or about February 7, 2024, while installing water lines for a new housing community, employees were exposed to soil collapse while working in a trench that had not been inspected by a competent person.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).  On or about February 7, 2024, while installing water lines for a new housing community, employees were exposed to soil collapse while working in a trench that was more than five(5) feet in depth with unprotected sides.   Jerlow Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1411703, citation number 1, item number 2, and was affirmed as a final order on November 7, 2019, with respect to a workplace located at 3100 Covell in Edmond, OK.  Jerlow Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1456511, citation number 1, item number 1, and was affirmed as a final order on March 5, 2020, with respect to a workplace located behind 14820 Serenita Ave. in Oklahoma City, OK.
