29 CFR 1910.36(b)(3): A single exit route was not permitted where the number of employees, the size of the building, its occupancy, or the arrangement of the workplace was not such that all employees would be able to evacuate safely during an emergency:  Note to paragraph (b) of this section: For assistance in determining the number of exit routes necessary for your workplace, consult NFPA 101-2009, Life Safety Code, or IFC-2009, International Fire Code (incorporated by reference, see �� 1910.6).  a) On or about February 21, 2020 employees required to exit the air traffic control tower were exposed to smoke inhalation and other fire egress hazards and the employer did not ensure the workplace was arranged with the smoke control damper maintained in the open position (louvers in the raised position) as designed for allowing the flow of smoke in the event of a fire.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work. They were not of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest was not securely clamped after each adjustment. The adjustment was made with the wheel in motion:  a) On or about January 21, 2020 in the building services support area the Wissota Grinder (Model# E-6, Serial# W4-82) was found with a work rest set at a distance of eleven-sixteenths of an inch exposing employees to struck-by hazards and the employer did not ensure work rests were kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.

29 CFR 1910.215(b)(9): Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section was exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  a) On or about January 21, 2020 in the building services support area the Wissota Grinder (Model# E-6, Serial# W4-82) was found with a tongue guard set at a distance of nine-sixteenths of an inch exposing employees to struck-by hazards and the employer did not ensure the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top did not exceed one-fourth inch.  Notice#1, Item#2, 29 CFR 1910.215(b)(9) as required by 29 CFR 1960.8(b): The employer failed to ensure floor pedestal grinder was equipped with safety guard adjusted to within one-fourth inch of the wheel. The employer was cited for a violation of the same standard on October 13, 2019, Inspection# 1419042, date November 26, 2019 of final order/settlement, and means of abatement.

(Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.)    a) On or about January 21, 2020 the safety data sheet for acetylene fuel gas was not maintained in the workplace exposing employees to chemical hazards and the employer did not ensure safety data sheets were maintained in the workplace.    b) On or about January 21, 2020 the safety data sheet chemicals were not readily accessible to employees exposing employees to chemical hazards and the employer did not ensure safety data sheets were readily accessible in the workplace.     Notice 2, Item 1, 29 CFR 1910.1200(g)(1) as required by 29 CFR 1960.8(b): The employer failed to ensure safety data sheets were available in the workplace. The employer was cited for a violation of the same standard on August 11, 2017 Inspection# 1237712, date September 6, 2017 of final order/settlement, and means of abatement.    Notice 1, Item 5b, 29 CFR 1910.1200(g)(1) as required by 29 CFR 1960.8(b): The employer failed to ensure safety data sheets were available in the workplace. The employer was cited for a violation of the same standard on September 25, 2017, Inspection #1256238, date October 19, 2017 of final order/settlement, and means of abatement.    Notice 2, Item 1, 29 CFR 1910.1200(g)(8) as required by 29 CFR 1960.8(b): The employer failed to ensure safety data sheets were available in the workplace. The employer was cited for a violation of the same standard on February 7, 2018, Inspection #1271159, date March 6, 2018 of final order/settlement, and means of abatement.

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted:  a) On or about January 21, 2020 at the administration building northwest exit door an exit sign was not illuminated exposing employees to emergency egress hazards and the employer did not ensure exit signs were illuminated.
