29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about February 12, 2020, the employer did not ensure employees were protected while performing cleaning activities at Station #11 Econoline. The employer did not ensure that the energy control application steps to control the electrical energy sources were implemented prior to the maintenance tasks the employer did not: � Shut down the equipment using the procedures established. � Physically operate energy isolating devices to control energy sources and affix lockout device(s). � Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. � Verify that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR 1910.146(g)(4): The employer's certification of the training required by 29 CFR 1910.146(g)(1) through 29 CFR 1910.146(g)(3) did not contain each employee's name, the signatures or initials of the trainers, and the dates of the training: a) The employer did not certify that employees who are tasked with work activities in and around permit required confined spaces had documented training. Employees tasked with work activities such as cleaning out the rinse tanks on the E-coat line (confined space) had training, however it was not certified to include the employees name, signature or initials of the trainer and dates of training.
