OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.   At the workplace located at 127 Claxton Avenue, Carthage, Missouri 64836, employees were exposed to struck-by, caught-in and amputation hazards while operating the Hayssen 1 Shred Johnson Cutter due to unexpected machine operation while body parts were inside of the point of operation and/or under the pneumatic guard.   Among other methods, one feasible and acceptable abatement method is to:  a) Guard the foot pedal to prevent unintended operation in accordance with American National Standard ANSI B11.3-2012.  b) Provide a two-hand method of actuation available for operator use.
