1910.147(c)(7)(i)(A) Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. At a facility located at 127 Claxton avenue, Carthage, MO 64836, employees were exposed to crushed by, caught in, struck by and amputation hazards. The employer exposed the employees to these hazards when employees entered areas of the PRO-PAL 3500 AST Palletizer without first utilizing appropriate lockout/tagout procedures to deenergize the machine. Instances of this exposure include but are not limited to: Instance 1: Employees access the upper and lower infeed areas of the palletizer machine without first locking out the pneumatic and the electrical systems of the machine. Instance 2: Employees accessed the enclosed area of the palletizer machine, through an opening near the area where pallets exited the machine, after breaking the light barrier system to stop the machine from operating. Employees did not utilize established lockout/tagout procedures prior to entering the area.
