29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Re-man area, Moulder machines 1-3; On or about 25 February 2020 and at times prior to, the employer exposed employees to potential unexpected energization of equipment in that employees were allowed to conduct set-up and tool change operations on moulder machines #1-3 without locking out the equipment. The employer allowed employees to rely on control circuitry (E-stop/key selector) as the energy isolating device which is not allowed by the standard. The employer failed to ensure employees utilized the machine specific lockout tagout procedures for those machines.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Re-man area, Moulder machines 1-3; On or about 24 February 2020 and at times prior to, the employer allowed employees to conduct maintenance and or servicing activities such as but not limited to jam clearing on moulder machines 1-3 which required the application of energy control without first training the employees on the safe application, usage, and removal of energy control devices exposing the employees to potentially hazardous energies.   b) Re-man area, Moulder machines 1-3; On or about 24 February 2020 and at times prior to, the employer allowed employees to conduct maintenance and or servicing activities such as but not limited to roller housing changeout on moulder machines 1-3 which required the application of energy control without first training the employees on the safe application, usage, and removal of energy control devices exposing the employees to potentially hazardous energies.
