29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):    (a) Reynolds Construction, LLC at 3555 Grant St. Denver, CO 80203: On or before 8/5/2020 the employer did not protect employees from injury in that a trench dug to replace water service lines was not equipped with a protective system in accordance with this standard. The trench had vertical walls and the soil type was C. The trench was 5.5 feet deep near the ladder and was 6.2 feet deep at its greatest depth. These conditions exposed employees to crushing and engulfment hazards.
