29 CFR  1926.651(b)(3):When excavation operations approach the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means.   On or about September 30, 2020, employer did not enforce the use of acceptable means to locate a gas utility line. The gas line was raptured while excavating. This condition exposed employees to burn hazards.
