1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a.)  Green Acres Contracting Company, Inc./I-80 Clearfield, PA  - On or about 10/22/20 and at times prior thereto,  an employee was riding unrestrained on the back of the box truck while it was being driven.    The employee leg was caught between the post for a guardrail and the lift gate which trapped the employees leg resulting in fractures.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
