29 CFR 1910.305(a)(2)(x):  Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  On or about November 11, 2020 employees were exposed to electrical shock and fire hazards in the following instances;  a) Workout trailer a flexible electrical cord was observed going through a metal wall with no electrical conduit to protect the card from damage.   b) Underneath trailer an electrical extension cord was observed being held in place by chicken wire.

29 CFR  1910.305(b)(2):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about November 17, 2020; employees were exposed to electrical shock and fire hazards in the following instances;  a) Behind the toolpushers trailer an outlet box was observed with a missing cover.    b) Heating Ventilation and Air Conditioning (HVAC) system was observed with a missing panel exposing electrical wires.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects :  On or about November 17, 2020; employees were exposed to electrical shock and fire hazards in the following instances;  a) Under Mud Engineer trailer #815; a broken conduit with exposed wires was observed.   b) Out side of the workout trailer was electrical cords daisy chained. flexible cords are not designed to be used in this way.
