29 CFR  1910.176(b):Storage of material created a hazard:  (a) Warehouse: On or about 12/17/2020, and at times prior, the employer exposed employees who were unloading steel beams from a flat bed truck to struck-by hazards in that there was not enough room available to place the load safely on the dunnage. The 10" x 10" steel beams were stored on approximately 4" of available dunnage.

Warehouse: On or about 12/17/2020, the employer did not report to OSHA within 24 hours the hospitalization of an employee resulting from a work related injury.
