On or about January 25, 2021, in the pre-op area, an employee mixed two incompatible chemicals in a container, resulting in a chemical reaction and exposing the employee's face and eyes.  The employer did not ensure the employee used protective safety glasses, goggles or faceshields. The employee was thereby exposed to eye injuries from sodium hypochlorite 12.5% and lactic acid 88%.
