29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a) Packaging Department - On or about September 2, 2022, authorized employees were required to perform servicing and/or maintenance on the Multivac Vaccum Packaging Machine #1 Model #R140 and other operating equipment. The employer did not ensure periodic inspections of the energy control procedures were performed at least annually as required by the standard.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  a) Packaging Department - On or about September 2, 2022,  the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices, thereby exposing employees to amputation hazards.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Packaging Department  - On or about September 2 2022, employees were exposed to machine hazards associated with moving parts when removing a jam on Multivac Vaccum Packaging Machine #1 Model #R140. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.
