29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace:  a.) Erie Forge & Steel, Inc.- On or about February 2, 2021, and at times prior thereto employer did not provide respiratory protection training for employees required to utilize RPB Radex Air Supplied  Abrasive Blasting Hoods during blasting operations.  b.) Erie Forge & Steel, Inc.- On or about February 2, 2021, and at times prior thereto employer did not provide respiratory protection training  for employees required to utilize 3 M Adflo Powered Air Purifying Respirator Welding Helmet during welding operations.   ERIE FORGE AND STEEL, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 29 CFR 1910.134(K)(3), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1351382 , CITATION NUMBER 1, ITEM NUMBER 1B AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 24, 2019, IN RESPECT TO A WORKPLACE LOCATED AT 1342 W 16TH STREET, ERIE, PA.  Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:   a.) On or about February 2, 2020, wipe samples were obtained from lunch room tables.  A cleaning schedule for the tables had not been implemented.    Analytical results indicated the presence of toxic metals, as follows:  1.) Table left table in Boring Department: 417.3ug (micrograms) Iron, 35.1ug Nickel, 6.4ug Copper Fume, 3.0ug Manganese Fume ; and   2.) Table Welding Department: 47.0ug (micrograms) Iron, 34.7ug Nickel, 13.8ug Copper Fume, 10.6 ug Chromium.  Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area:  a) Erie Forge & Steel, Inc.- On or about February 2, 2021, and at times prior thereto, employees worked with and around various types of hazardous chemicals and substances including, but not limited to, Epoxy Resin, Carbon monoxide,  and metal dust. The company did not ensure employees were provided with effective information and training on the physical and health hazards of the chemicals in the work area.  ERIE FORGE AND STEEL, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 29 CFR 1910.1200(H)(3)(II), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1351382 , CITATION NUMBER 1, ITEM NUMBER 3A AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 24, 2019, IN RESPECT TO A WORKPLACE LOCATED AT 1342 W 16TH STREET, ERIE, PA.
