29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about February 18, 2021 or prior to: Employees working on Line 1 of the production area are exposed to amputation hazards where they manually remove or slide the bread down the slide when it gets stuck. Adequate guarding of the pinch point on the SASIB Model # 550-58-3, SN: 999310-A975, Line 1, Lateral Transfer Conveyor was not in place.  On 2/10/21 an employee's right index finger tip was amputated when removing stuck bread from the slide plate area.

29 CFR  1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.    On or about February 18, 2021 and prior to: Employees working on Line 1 in the production area are exposed to amputation hazards where they were performing service, such as but not limited to, troubleshooting, and removing stuck bread from the slide plate of Line 1 lateral transfer conveyor.  The employer did not provide Lockout / Tagout training on the purpose and function of the energy control program.  .
