29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a) On or about 02/20/2021 - 8785 Segers Trail Loop, Lot #13,  Madison, AL, an employee conducting framing work in the house attic with a  hatch access door (40" x 22.5") was not protected from falling approximately 12.5 feet to the ground below.
