29 CFR 1910.147(c)(4)(ii)(B):The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) Facility: On or about March 10, 2021, a maintenance employee was working on the K Plant Sacker machine and it was not electrically locked out. In the lockout/tagout energy control procedure for that K Plant Sacker, (that was updated on 3/9/21) step 3 reads as follows: 3. In the circuit breaker panel beside the bulletin board, locate Breaker #26 and turn it off. Apply a Circuit Breaker Lock to this breaker and attach a lock and tag. This Breaker #26 did not supply any electrical power to the K Plant Sacker.

The employee dumped about 1,800 pounds of mix (dolomite and RHT100 oil? down and through the BL sacker.
