29 CFR  1910.147(c)(4)(ii)(B):The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  (a)  Facility:  On or  about March 10, 2021, a maintenance employee was working on the K Plant Sacker machine and it was not electrically locked out.  In the lockout/tagout energy control procedure for that K Plant Sacker, (that was updated on 3/9/21) step 3 reads as follows:  3.  In the circuit breaker panel beside the bulletin board, locate Breaker #26 and turn it off.  Apply a Circuit Breaker Lock to this breaker and attach a lock and tag.  This Breaker #26 did not supply any electrical power to the K Plant Sacker.

29 CFR  1910.147(c)(9):The employer or authorized employee did not notify affected employees before the lockout or tagout devices were applied to the machine or equipment:  (a)  Facility:  On or about March 9th and 10th, 2021, the employee that was operating the BL Mixer located above the BL sacker was not notified that the BL sacker was being worked on.  The employee  dumped about 1,800 pounds of mix (dolomite and RHT100 oil? down and through the BL sacker.
