This condition allowed the handle (including the lock) to come off of the electrical panel to the machine, thus allowing the steel knob (that the handle was affixed to )to be turned and activate (supply electrical power) to the moving robot.

29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)  (a)  Facility:  On or about 10/23/2021, a Toyota forklift (L5) was leaking LPG and the forklift was not taken out of service.  This L5 Toyota forklift continued to be used after management was told it was leaking LPG.  In addition, this leak was not repaired in accordance with section 4.1.3.10 Fuel tank Check of the Toyota Operators and Owners Manual, which calls for the Toyota dealer to do fuel repairs to prevent a fire or explosion.    (b)  Facility:  On or about 10/23/2021, the Toyota forklift (L4) had a fuel guage that was not working.
