29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee 50 Leo M Birmingham Highway, Boston, MA On or about, March 18, 2021, personal fall arrest equipment such as hinged roof anchors and vertical lifeline rope grab systems, were not capable of supporting at least 5,000 pounds and were not designed as part of a complete personal fall arrest system, which maintained a safety factor of at least two, by a qualified person. Northeast Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(15), which was contained in OSHA inspection number 1317338, citation number 01, item number 04 and was affirmed as a final order on 01/31/2020, with respect to a workplace located at 191 Paris Street, East Boston, MA 02128

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) 50 Leo M Birmingham Highway, Boston, MA On or about, March 18, 2021, employee exposed to falls from the second floor through fourth , using personal fall systems, were not trained by a competent person qualified in the following areas: the correct procedures for erecting, installing, maintaining, disassembling, and inspecting the fall protection systems to be used; the use and operation of personal fall arrest systems and other protection to be used; and, the standards contained in this subpart. Northeast Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(2), which was contained in OSHA inspection number 1317338, citation number 01, item number 05b and was affirmed as a final order on 01/31/2020, with respect to a workplace located at 191 Paris Street, East Boston, MA 02128
